We use a referral form that is uniform throughout the school. It
automatically is made out with four copies (one for the referring teacher,
one for the homeroom teacher, one for administration, one for parents).
Although the music teacher might not know that I have made a discipline
referral on a student, the homeroom teacher,administrators, and parents do.
When it comes time conference with parents, they cannot say we have "teamed
up" on their child.

The form itself is simple and includes the district discipline policy. We
simply need to check the area(s) of offense, note what we did for punishment,
and send the form on its way. When a certain number of forms are gathered on
one child, an administrator and/or the teachers call the parent and a
conference is scheduled. D-hall can not be assigned without a parent
conference, but this is simply to insure that parents are aware of the
situation and that kids have transportation home on the day of the detention.
