This is the one I use: http://www.kickinit.com/cgi-bin/nslookup.cgi
It's simple and straightforward, though sometimes it comes up empty.
Send the complete headers and message to abuse@the ISP from which the
spam originated. Spammers often originate or route through foreign
countries' ISPs, which don't seem to have abuse departments. In that
case, address it to postmaster@...
