Hey Christy,
I'm a second year teacher in a pre-k to 5th grade elementary school. My
discipline plan:
1-verbal warning
2-"time out" or away from the group
3-OOOPS! letter sent home to parents/guardian. This is a form letter
addressed to and individual parent or guardian. The student fills out what
they were doing during class. He/she takes it home, has it signed, and
brings it back the next day. I then file it and use it as a record of
4-office referral

It may not be the most creative plan, but it works with my students. They
seem to take more responsibility for their actions when they have to explain
what they were doing wrong.
