Lesson Format I use:

 Discussion/Introduction
 Objectives/Standards Based
 Production Process (if applicable)
 Materials
 Vocabulary
 Assessment
 Enrichment/Extensions

This is used by all the visual arts teachers in our district. It is pretty thorough.
