I teach at an alternative middle school where the kids go after they
have been put out of their school for continuous class disruptions.
Something you might try that often works for me is to ask the student
that is giving you the most problems, to be your assistant. Put them in
charge of something, clean-up is a good one, and give them a huge amount
of praise when they are helpful instead of disruptive. If this helps,
then send a note home w/ the student, or call the parent and let them
know how good their child has been, most will be surprised and overjoyed!
