Hi, Peggy--

I thought maybe you decided not to do the project. I am cutting and pasting
a copy of the questionnaire I have used in the past and it does cover personal
interests, goals, etc. I can make sure that my students include this in
their portraits of your students.

Since you see you kids only once a week, does this project fit your schedule?
 If there is a shorter project you would like to do, let me know. It looks
like it may take you several weeks to complete the portraits, unless you see
students for an extended period of time each week.

Anyway, here is my questionnaire:

can. USE COMPLETE SENTENCES!

What is the shape of your face? Describe it as completely as possible. Be
sure to describe the shape, length, and width of your cheeks, chin, and
forehead, too.

How would you describe your skin color and tone? (Fair, dark, rosy, ruddy,
pale, etc.)

What color are your eyes?

What is the size and shape of your eyes?

What color are your eyebrows? Are they lighter or darker than your hair or
the same color? How would you describe their thickness and shape and arch?
Are they close to your eyes or high above your eyes?

How would you describe the size and shape of your nose?

How would you describe the size and shape of your mouth? Be very specific
about the width of your mouth and how each lip is shaped.

Do you usually smile? What is your most common expression? (Happy, sad,
bored, surprised, tired, excited, frightened, etc.)

How would you describe your teeth? Do you wear braces?

What is the color, thickness, length, and texture of your hair?

Describe in detail the way you wear your hair.

Describe the size and shape of your ears.
If you are a female, do you wear makeup? Describe the makeup you use, the
colors, and how you use it. Do you wear any jewelry, glasses, or hair ornaments
that should be included in a portrait? If so, describe them. (If you wear
glasses, describe the size, shape, and color of the frames.)

Do you have freckles, moles, dimples, laugh lines, scars, etc. that are
noticeable and/or make your face unique? If so, where are they on your face?

Describe your favorite shirt or sweater. What kind of clothing do you like?

What are your three favorite colors?

What are your hobbies and interests?

What five words describe your personality?

Is there anyone that people say your remind them of? If so, who?

Finish this sentence: A portrait of me should (be, include,
