Bunki,
We start a new block term in January. Could I have copy of your form so I
can use it as I work up one for my new students? We have a blanket form in
our agendas which the student and parent must sign. It really is intended
for using the internet with an addendum for having a photo used. We only
use first names if a photo of the student is used. There is no provision
for art work on the web.

My form covers all plus explains what will be exposed on the web and the
protection the school provides. If you would like to see my form, let me
know. It's not a biggie and so easy to do at the beginning of the year to
